Eyelid Surgery Information

Various procedures can be undertaken to improve the skin around the eyes. Eyelid surgery (blepharoplasty), is a relatively short procedure with minor pain and brief recovery process. Below are some common concerns relating to the eyes and the surgery techniques used to repair damage:

Drooping or hooded eyelids

Upper eyelid blepharoplasty is used to remove excess skin and fat and/or reposition and tighten muscles. The results provide a smoother and more alert appearance. An incision is made along the natural fold of the eyelid followed by removal of a thin section of muscle, unnecessary skin and any surplus fatty tissue. Fine removable or absorbable stitching is used to close the wound. The length and shape of the incision will vary depending on the amount of excess skin.

Under-eye bags & loose skin from fatty deposits

Options for correcting puffy or baggy skin under the eyes:

  • Transconjunctival lower eyelid blepharoplasty – removal of fat through an incision inside the lower eyelid
  • Removal of skin/fat through an incision made on the outside of the eyelid just below the lash line
  • Lifting fatty pads which have slipped downward, either through an external incision just below the lower eyelashes or endoscopically (called the lower eyelid SOOF technique)
  • Manipulating or reducing the eyelid muscle, exposing the fat pockets and removing the excess by lifting away all of the eyelid skin (lower eyelid skin flap method)
  • Horizontal tightening through an incision in the upper eyelid, creating an upward slope.

How is Blepharoplasty Performed?

Excess skin can only be removed through external incisions. However, the internal incision method leaves no visible scarring and allows the supporting structures to remain intact. The wound is closed with dissolvable stitches and may even be left to heal naturally. The internal incision procedure is suitable for younger patients with relatively elastic under-eye skin and muscle.

If fine wrinkles remain following surgery, they can be treated with a chemical peel or laser skin resurfacing.If the lower lid is severely sagged or lax, then the surgeon may recommend canthopexy. This procedure tightens and elevates the lower lid by using one or two stitches (dissolvable or permanent) to attach the muscle to a higher position on the outer bony rim. It lengthens the blepharoplasty operation by about half an hour and does not require any additional scars. Alternatively, canthoplasty may be used to reposition and reinforce the lower eyelid by cutting and permanently repositioning the tendons.

Post Operation

Following surgery, vision should be monitored for several hours. You should be rested with the head elevated above the heart level to avoid any bleeding complications. The eyes will be puffy and swollen with bruising. You will require assistance for the first 24 hours. Ice packs and cool compresses will help the swelling and bruising. You will most likely be given oral medications as well.Bleeding may occur even after three weeks. You must keep the head elevated and not sleep on the eyelids for at least three weeks or in accordance with your surgeon’s instructions. Bending forward for any reason should also be avoided.Contact lenses cannot be worn following surgery until the surgeon advises so. Post surgery the eyes feel dry and are sensitive to bright light. Eye drops should be used in order to keep the eyes lubricated and ointment will keep your incisions cleansed.

Risks

As with all procedures, bleeding and infection can occur. Other risks include ectropion (development of a pull-down of the lid), dry eye, muscle injury, cysts, scarring, prolonged bruising, poor healing, corneal abrasion and blindness.As there are serious risks involved, you should always look to a qualified surgeon with experience in the procedure you are considering before undergoing any surgery.
